Please Note: No requests will be considered without a minimum of two weeks' notice prior to the date you are requesting. Currently, all room reservation requests are being reviewed on a case-by-case basis due to ever-changing safety protocols and restrictions.

I UNDERSTAND AND AGREE TO THE FOLLOWING: *

All guests are required to wear a mask whenever they are inside any of FUMCMC's buildings.
I will communicate and help enforce the mask policy to the attendees at my event.
All room set-ups must honor the current social distancing & Covid guidelines (6ft apart) set by FUMCMC and/or the government. If I set up the room, I will adhere to those policies.
We will be respectful of the facilities at FUMCMC and will clean up any messes, return any furniture or items to the location we found them, will take out any trash we made to the dumpster, and leave the room in the same, if not better, condition as we found it.
We do not provide tablecloths or any room decorations with a room rental. If you are interested in renting table linens, then please contact the Church Office.
If we choose to decorate the space, we agree to ONLY use painters tape to attach any items to walls, ceilings or anything else in the room. No push pins, duct tape, staples, packaging tape, or any other obtrusive items can be used.
With submitting this form, I recognize this is only a request. Formal approval will be sent to my email.
